All Committee members are asked to provide a complete list of interested optical/optometric parties/businesses in which they have either a commercial interest either because they, or a member of their immediate family, own part or all of the business or because they or a member of their immediate family are employed by them. In addition they must provide a list of all of the organisations for whom they work in any capacity even if not as an employee. E.g. committee work or as a consultant.
